We confirm with a combustion-gas test on the coolant, cylinder leak-down and pressure testing. When the head does come off, it goes to a machine shop for checking and resurfacing, and we use updated multi-layer steel gaskets.

Rarely, and they often clog the heater core and radiator. We don't recommend them.
Is it always the head gasket?
No — we test first, because the fix for a cracked head is very different.
